Description of Assistance
I/we, the undersigned, understand that AVL City Church, a nonprofit organization, is providing assistance to homes affected by the Hurricane Helene flood. The assistance provided may include but is not limited to cleaning, demolition, debris removal, construction, distribution of supplies, or other forms of disaster relief work at my/our property.
Acknowledgment of Risk
I/we acknowledge and understand that receiving disaster relief assistance involves certain risks, including but not limited to:
Potential property damage
Personal injury to myself/ourselves or others on my/our property
Exposure to potentially hazardous materials or environments
I/we further acknowledge that AVL City Church, its employees, leaders, agents, and volunteers are not responsible for pre-existing conditions or risks inherent to disaster recovery work.
Assumption of Risk
I/we voluntarily agree to allow AVL City Church and its volunteers to provide assistance at my/our property, fully aware of the risks involved. I/we assume full responsibility for any injury or damage that may result from their activities.
Waiver of Liability
In consideration of receiving assistance from AVL City Church, I/we hereby release, discharge, and hold harmless AVL City Church, its directors, officers, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or in connection with the relief activities at my/our property, including but not limited to injury, illness, damage, or loss resulting from any cause whatsoever, including negligence.
Damage to Property
I/we understand that AVL City Church and its volunteers will exercise reasonable care while providing assistance; however, I/we acknowledge the possibility of unintentional damage to my/our property due to the nature of disaster recovery work. I/we waive any claims for compensation for property damage arising from these activities.
Medical Treatment
I/we consent to receive medical treatment, if necessary, during the relief activities and release AVL City Church from any claims related to medical care, including inadequate or delayed treatment.
Indemnification
I/we agree to indemnify and hold harmless AVL City Church and its volunteers from any claims brought by third parties arising from the relief work performed at my/our property.
Photo and Media Release
I/we grant AVL City Church permission to use any photographs, videos, or other media taken during relief work at my/our property for promotional, educational, or reporting purposes without compensation.
Agreement and Signature
I/we have read this Liability Release Form, fully understand its terms, and sign it freely and voluntarily. I/we understand that by signing this form, I/we waive certain legal rights.
By signing this document electronically I hereby release from liability and agree to hold harmless the volunteers for any damage or injury that may occur on my property, to any of my property or to my person, which may occur during the cleanup operation. I further understand and agree that there is no warranty, implied, written or oral, for any work performed on my property by said volunteers. I understand that the AVL City Church is a volunteer organization that has limited volunteers, limited financial and material resources, and makes no guarantee that said service will be provided. Additionally, I further understand that THIS IS NOT A CONTRACT TO PROVIDE SERVICES.
